Both are the most clinically validated OTC acne treatments available. But they work through completely different mechanisms — and work best for different types of acne.

How Salicylic Acid Works

Salicylic acid (BHA) is oil-soluble, meaning it penetrates into the sebaceous follicle. Once inside the pore, it:

  • Dissolves the keratin plugs that form blackheads and whiteheads
  • Reduces sebum production over time
  • Has mild anti-inflammatory effect

Best for: Blackheads, whiteheads, clogged pores, comedonal acne. Also excellent for general pore congestion in oily skin.

How Benzoyl Peroxide Works

Benzoyl peroxide is a bactericidal agent that releases free radicals inside the pore, directly killing C. acnes (the bacteria responsible for inflammatory acne). It:

  • Kills acne bacteria — no resistance issues (unlike antibiotics)
  • Reduces inflammatory papules and pustules
  • Does not penetrate pores as deeply as salicylic acid

Best for: Inflammatory acne — red pimples, pustules, cysts with a head. Not ideal for blackheads.

Can You Use Both?

Yes — they complement each other well. Use salicylic acid as a daily toner or serum to keep pores clear, and benzoyl peroxide as a spot treatment or targeted wash for active inflammatory breakouts.

Never layer them simultaneously in the same step — the combination increases irritation without proportional benefit.

Starting Concentrations

  • Salicylic acid: Start at 0.5–1%. Work up to 2% if well tolerated.
  • Benzoyl peroxide: Start at 2.5% — just as effective as 10% for most people, much less irritating. Reserve 5–10% for resistant cases.
